The certificate also verifies that he suffered from coronary artery disease and Parkinson’s disease, which contributed to his death. The causes of death are listed as follows: “a) Cardiac arrest outside of hospital; b) acute heart attack; c) coronary artery disease and Parkinson’s disease with autonomic dysfunction (joint causes).” The same document lists Osbourne’s profession as “performer, composer, and rock legend.”

According to the New York newspaper, on the day he died, an air ambulance flew the singer about 12 kilometers from his home near the village of Chalfont St. Giles in Buckinghamshire to Harefield Hospital in the London suburb of Uxbridge.

On July 22, his family announced his death without providing details: “It is with indescribable sadness that we announce that our beloved Ozzy Osbourne passed away this morning. Black Sabbath's last concert

Osbourne performed at what was announced as Black Sabbath’s final show a few weeks before his death. It took place at a festival in Birmingham, England, the hometown of both the singer and the band. Also on hand were artists such as Ronnie Wood of the Rolling Stones and Steven Tyler of Aerosmith. Metallica, Guns N’ Roses, and Slayer also performed. “It’s been six years since I’ve been in bed, and you have no idea how I feel.
